Procedure Overview

This family describes moving a bone of the skull, face, chest wall, or upper extremity back to its normal anatomic location, or to another location that will function better, without cutting the bone free and removing it from the body. It's the code family used for reducing a displaced fracture, correcting a malunion, or realigning a bone that has shifted out of its normal position due to trauma or a congenital condition.

The procedure preserves the bone's own blood supply and continuity while adjusting its position - fixation with pins, plates, or screws is often part of the same operative episode to hold the corrected alignment in place.

Thoracic Vertebra

The thoracic vertebrae anchor the rib cage and support the mid-back, and their alignment influences both spinal curvature and the space available for the spinal cord as it passes through this longer, less mobile segment of spine. Reposition here is most often performed for structural scoliosis or kyphosis, traumatic fracture-dislocation, or vertebral displacement from tumor or infection that has weakened the supporting bone and ligaments. Correcting thoracic vertebral position is technically demanding because the vertebrae are connected to the ribs and because the spinal canal at this level is narrower relative to the cord, leaving less margin for error during realignment. As with other spinal reposition procedures, instrumented fixation is generally placed at the time of correction to prevent the vertebra from settling back into its abnormal position.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
