Procedure Overview
Reposition procedures move a respiratory structure to its correct anatomic location without cutting any of it away or adding replacement material. The classic example in this body system is diaphragmatic plication, where a surgeon folds and sutures an abnormally high, flaccid diaphragm, often caused by phrenic nerve injury, back down into its normal position to improve breathing mechanics. Correcting a displaced or twisted lung lobe (torsion) after surgery also belongs here.
These procedures are chosen when the underlying tissue is intact and functional but sitting in the wrong place, as opposed to situations where tissue must be removed or reconstructed. The goal is purely mechanical realignment so the structure can do its normal job again.

Upper Lung Lobe, Right
The right upper lung lobe is the most superiorly positioned of the right lung's three lobes, sitting above the horizontal fissure and occupying the apex of the thoracic cavity near the clavicle. Reposition of this lobe is reported when the entire lobe is surgically moved to a different location within the body, such as in cases of diaphragmatic hernia repair where herniated lung tissue is returned to its normal thoracic position, or during correction of lobar torsion where the lobe has rotated on its bronchovascular pedicle. This differs fundamentally from resection or lobectomy, since the lobe itself remains intact and is not removed, and documentation should reflect that the intervention corrected the lobe's anatomic position rather than excising diseased tissue.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
Coding & Documentation
Coders need documentation that the surgeon relocated an existing body part to a normal or otherwise suitable position, distinct from simply repairing a hole or defect in place. For diaphragm cases, the note should describe plication or folding to reduce an eventration, not just suturing a tear. A common error is coding diaphragm plication as Repair because both involve suturing, when the defining action is repositioning redundant tissue rather than closing a defect.
