Bilateral orchiectomy removes both testes in a single operative episode, an intervention performed for androgen-deprivation therapy in advanced or metastatic prostate cancer, for bilateral testicular malignancy, for severe bilateral trauma, or, in select cases, as part of gender-affirming surgery. Removing both glands eliminates the primary source of testosterone production, producing a surgical castration effect that is permanent, in contrast to reversible pharmacologic androgen suppression, and this hormonal consequence is central to why the bilateral procedure is chosen specifically for prostate cancer management. The technique still generally proceeds through bilateral inguinal or scrotal incisions with cord ligation on each side. Because the physiologic and counseling implications differ substantially from a unilateral procedure, bilateral orchiectomy is documented distinctly rather than as two separate unilateral codes.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
