The right tunica vaginalis is the serous membrane derived from peritoneum that envelops the anterior and lateral surfaces of the right testis, and its complete resection is performed most commonly for hydrocele that recurs after simpler drainage or plication procedures, for testicular tumors where the membrane is involved, or occasionally for chronic hematocele. Excising this sac-like layer removes the space in which fluid accumulates, which is the underlying goal in hydrocelectomy performed by full excision rather than eversion. Because the tunica lies immediately adjacent to the testis and epididymis, the surgeon must dissect it free without compromising testicular blood supply, and hemostasis along the cut edge is important since this layer is vascular. Approach: Percutaneous Endoscopic
