Correcting, to the extent possible, a portion of a malfunctioning device or the position of a displaced device

Procedure Overview

This family covers procedures that correct a problem with hardware already implanted in one of the upper bones - the skull, facial bones, sternum, ribs, or the cervical and thoracic vertebrae, scapula, clavicle, or humerus. A patient who had a plate, screw, rod, or other fixation device placed after a fracture or spinal surgery may develop a loose screw, a bent plate, or hardware that has shifted out of its intended position. Revision procedures adjust, tighten, reposition, or partially rebuild that existing device rather than removing it and starting over.

Surgeons turn to these procedures when imaging or symptoms - pain, instability, a visible or palpable lump, or a healing fracture that isn't aligning properly - point to a hardware problem rather than a new injury. The goal is to restore the device to proper function or position using the smallest intervention that will do the job, which often means less recovery time than a full hardware exchange.

Carpal, Right

The right carpal bones - scaphoid, lunate, triquetrum, pisiform, trapezium, trapezoid, capitate, and hamate - form a tightly packed mosaic that transmits load between the forearm and hand while permitting wrist motion. Revision procedures here address a previously placed device, most often a screw, plate, wire, or partial wrist arthroplasty component used to stabilize a fracture, nonunion, or post-traumatic collapse such as scaphoid nonunion advanced collapse. Because the carpal row has minimal soft tissue coverage and depends on precise alignment for grip strength and wrist kinematics, revision may involve correcting hardware malposition, replacing a loosened or broken component, or adjusting a construct that has caused impingement or persistent instability. The small size and irregular shape of individual carpal bones make imaging-guided placement and documentation of which specific bone was revised especially important for coding accuracy.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

Device: Synthetic Substitute

Synthetic Substitute designates a device made from manufactured, non-biologic material, such as mesh or prosthetic components, used to replace or augment a body part. It is distinguished from the two tissue substitute categories by its artificial composition, which carries different considerations for integration and long-term durability than biologic grafts.

Coding & Documentation

A coder should look for documentation that names the specific device involved (plate, screw, rod, wire, external fixator component) and describes what was done to it - repositioned, tightened, partially replaced, or otherwise corrected - rather than fully removed and swapped. Operative notes should specify the exact upper bone treated, since the qualifier and body part values differ by site.

The most common assignment error is confusing Revision with Removal followed by Insertion when the surgeon actually took the whole device out and put in a brand-new one; that scenario is coded as separate root operations, not Revision. Another frequent slip is coding Revision when the note describes a routine Change of an identical device without any correction of malfunction or malposition - that belongs under Change instead.

Commonly Confused With

ChangeChange (0P2) is easily mixed up with Revision, but Change swaps a device for an identical or similar one through an existing opening with no correction implied, while Revision specifically fixes a malfunctioning or malpositioned device.
RemovalRemoval followed by Insertion is used instead of Revision when the original device is discarded entirely and a new one is put in its place.
RepairRepair is sometimes confused with Revision when no device is involved at all - Repair addresses the bone itself, such as a nondisplaced fracture line, not hardware.
