Procedure Overview
Revision procedures correct a problem with a previously placed device in the ear, nose, or sinuses, addressing either a malfunction of the device itself or a device that has shifted out of proper position. Cochlear implant revision surgery, performed when the internal receiver or electrode array has failed, migrated, or become infected, is a common example, as is repositioning or adjusting a nasal septal button or stent that has moved from its intended location. The underlying anatomy is not the target of the procedure - the device is.
These procedures are usually triggered by symptoms that signal device trouble, such as loss of hearing benefit from a cochlear implant, exposure or displacement of a nasal implant, or imaging findings confirming a device is out of place. Depending on what's found at surgery, revision may involve adjusting the device in place or, if it can't be salvaged, removing it, which would then be coded separately as Removal rather than Revision.

A common error is coding Revision when the device was actually fully exchanged for a new one, which changes the appropriate root operations entirely.
