The goal is to restore the device to proper function or position using the smallest intervention that will do the job, which often means less recovery time than a full hardware exchange.

The right finger phalanges - proximal, middle, and distal, across the index through little fingers - are slender bones that enable the fine, independent digit motion required for precision grip and manipulation. Revision procedures in this region commonly address hardware from a prior fracture repair, such as a Kirschner wire, screw, or miniature plate placed for a spiral, oblique, or intra-articular fracture, when that hardware has loosened, migrated, or led to stiffness from tendon adhesion. Because the phalanges are small and closely associated with the flexor and extensor tendon mechanisms, even minor hardware prominence can restrict joint motion or cause tendon irritation, prompting removal or exchange of the original device. Revision may also involve a previously placed interphalangeal joint implant. Given the number of phalanges per hand, documentation should specify the digit and phalangeal segment involved.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

The most common assignment error is confusing Revision with Removal followed by Insertion when the surgeon actually took the whole device out and put in a brand-new one; that scenario is coded as separate root operations, not Revision. Another frequent slip is coding Revision when the note describes a routine Change of an identical device without any correction of malfunction or malposition - that belongs under Change instead.
