Lower Extremity, Right

Revision of the right lower extremity applies to correcting or adjusting a previously placed device or a malfunctioning prior procedure within this broad anatomical region, rather than to the leg's native tissue itself. This body part designation is used when the device or complication cannot be tied to a single named structure lower in the body part hierarchy, such as adjusting a displaced external fixation frame spanning multiple segments of the limb, correcting malposition of hardware after a complex reconstructive procedure, or addressing a complication involving more than one anatomic layer of the extremity. Because the region is defined broadly, precise documentation of what device or prior procedure is being revised, and why, is essential for correct coding, since a revision confined to a single named structure like the femur or knee joint would instead be coded to that specific body part.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. A frequent mistake is coding Revision when the surgeon actually removed the old device and inserted an entirely new one, which typically calls for separate Removal and Insertion or Replacement codes instead; another is overlooking Revision when a fixation pin is simply repositioned during an encounter otherwise focused on a different procedure.

Commonly Confused With

RemovalRevision is easy to confuse with Removal, but Removal takes a device out without putting a corrected or new one in its place, while Revision adjusts a device that remains implanted.
