Operation Definition
Correcting, to the extent possible, a portion of a malfunctioning device or the position of a displaced device
Procedure Overview
Revision procedures correct a problem with a previously placed device in the ear, nose, or sinuses, addressing either a malfunction of the device itself or a device that has shifted out of proper position. Cochlear implant revision surgery, performed when the internal receiver or electrode array has failed, migrated, or become infected, is a common example, as is repositioning or adjusting a nasal septal button or stent that has moved from its intended location. The underlying anatomy is not the target of the procedure - the device is.
These procedures are usually triggered by symptoms that signal device trouble, such as loss of hearing benefit from a cochlear implant, exposure or displacement of a nasal implant, or imaging findings confirming a device is out of place. Depending on what's found at surgery, revision may involve adjusting the device in place or, if it can't be salvaged, removing it, which would then be coded separately as Removal rather than Revision.
Anatomy & Axis Detail
Nasal Mucosa and Soft Tissue
Nasal mucosa and soft tissue as a body part encompasses the lining and underlying soft structures of the nasal cavity apart from the turbinates and septal cartilage or bone specifically, and it is commonly involved in procedures for chronic rhinitis, polyps, or reconstructive nasal surgery. Revision addresses complications of an earlier intervention on this tissue, such as recurrent synechiae or scar bands that have reformed after septoplasty or turbinate reduction, a mucosal flap that has partially failed, or persistent crusting and stenosis following prior soft tissue surgery. Because nasal mucosa is highly vascular and prone to adhesion formation during healing, revision often involves lysis of scar tissue and placement of a spacer or stent to maintain the airway while the corrected area heals.
Approach: External
External approach applies to procedures performed directly on the skin or mucous membrane, or on an accessible body surface, without any instrumentation passing through a puncture or orifice. It covers things like manual reduction of a fracture or excision of a skin lesion. It differs from Via Natural or Artificial Opening in that no internal passage is entered at all, only the exposed surface.
Device: Nonautologous Tissue Substitute
Nonautologous Tissue Substitute refers to biologic material sourced from a donor or another species, such as an allograft or xenograft, used to replace a body part. It differs from Autologous Tissue Substitute by tissue origin outside the patient's own body, and from Synthetic Substitute by being biologic rather than manufactured material.
Coding & Documentation
Revision is coded when documentation shows the surgeon corrected a malfunctioning device or repositioned a displaced device without taking it out entirely; if the device is instead taken out and a new one put in, that's a Removal followed by an Insertion or Replacement, not Revision. Coders need the operative note to specify what was wrong with the device (mechanical failure, migration, extrusion) and what corrective action was taken, since vague notes just saying 'device revised' leave the specific correction unclear.
A common error is coding Revision when the device was actually fully exchanged for a new one, which changes the appropriate root operations entirely. Coders should also watch for cases where only part of a multi-component device, such as one electrode of a cochlear implant array, was adjusted, since the device value coded should reflect what's documented as revised.
