Procedure Overview

Revision procedures address a gastrointestinal device that is no longer working the way it should, or has shifted out of its correct position. This covers a wide range of situations, from adjusting or replacing components of a gastric band to repositioning a feeding tube, colostomy appliance anchor, or sphincter device that has migrated after the original placement. The goal is to fix the existing device to the extent possible rather than remove it outright and start over.

These operations are usually driven by a complication of an earlier procedure - erosion, slippage, leakage, or mechanical failure of hardware such as a band, port, or reservoir. A surgeon may tighten, loosen, reposition, or partially replace parts of the device during the same operative session, often using imaging to confirm the device's location before and after the correction.

Because revision responds to a problem with something already implanted, it is closely tied to the patient's surgical history, and documentation of the original device and the nature of its malfunction guides the current treatment plan.

Omentum

The omentum is the fatty, well-vascularized peritoneal fold draping from the stomach over the abdominal viscera, frequently mobilized during earlier operations as a pedicled flap to buttress an anastomosis, patch a perforation, or fill a surgical dead space. Revision procedures on the omentum address problems with material or a device left behind from that prior use, such as a displaced mesh, a malpositioned flap, or a leaking omental patch that no longer seals the defect it was meant to cover. Because omental tissue is soft and mobile, revision may involve repositioning the flap, tightening or loosening its attachment, or removing degraded suture material, and documentation should specify what was corrected and why the original repair had failed rather than simply noting that omental tissue was touched again.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

Device: Autologous Tissue Substitute

Autologous Tissue Substitute identifies a device value where material harvested from the patient's own body, such as an autograft, is used to replace or reconstruct a body part. Its defining feature is tissue origin from the patient, setting it apart from Nonautologous Tissue Substitute, sourced from a donor or animal, and Synthetic Substitute, made of manufactured material.
