Procedure Overview

Supplement procedures add biological or synthetic material to reinforce or augment a lower joint that is still structurally present but needs additional support. Ligament reconstruction, such as an ACL or PCL graft in the knee, is the classic example, along with labral augmentation in the hip or cartilage grafting. The right ankle joint, the tibiotalar articulation bearing the body's full weight during stance and gait, is supplemented when its native cartilage, capsule, or ligamentous restraints have degraded beyond simple repair but a joint-replacing device is not warranted. A biological or synthetic patch, autologous cartilage graft, or ligament augmentation is placed to reinforce the talar dome or malleolar mortise, restoring the tight congruity this joint depends on for stable dorsiflexion and plantarflexion. Because the ankle mortise tolerates very little laxity before developing early arthritis, surgeons documenting this procedure specify which surface or ligament received the supplementing material, and whether the graft was autograft, allograft, or a nonbiologic substitute, since the device type changes the code's qualifier.
