Papillary Muscle

The papillary muscles are conical muscular projections from the ventricular wall that anchor the chordae tendineae and coordinate valve closure with ventricular contraction; they are vulnerable to ischemic necrosis after myocardial infarction, which can lead to rupture or dysfunction and severe valve regurgitation. Supplementing a papillary muscle typically means reinforcing a weakened or partially disrupted muscle head with a patch, pledget-supported suture buttress, or synthetic material to restore its structural integrity and preserve its tethering function for the valve leaflets, rather than replacing the muscle entirely. This is distinct from chordal replacement and is performed when the muscle body itself, not just its cords, needs reinforcement. Given the papillary muscles' deep intraventricular location, this procedure is almost always performed as part of open-heart valve repair, and documentation should clarify whether the anterolateral or posteromedial muscle group was involved.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
