This is the coding family for pedicled skin flaps, where a flap of skin and underlying tissue is rotated or advanced into a nearby defect while remaining tethered to its original blood vessels. It is commonly used to cover wounds, pressure ulcers, or defects left after tumor removal in areas where a local flap can reach.

Surgeons choose flap transfer when a wound is too large or too deep for simple closure or a skin graft alone, and when preserving blood flow through a pedicle offers better healing and durability than a free graft. The flap is shaped and positioned so it functionally substitutes for the tissue that was lost, and the donor site is typically closed directly or with a separate graft.

Transfer of scalp skin moves a flap of skin, still attached to its native blood supply, from one location on the scalp to an adjacent defect, commonly used to close wounds from tumor excision, trauma, or chronic non-healing ulcers where the surrounding scalp lacks enough laxity for simple closure. The scalp's dense subcutaneous layer and rich vascular network make local flaps a reliable option, and rotation or advancement flap designs are frequently used to preserve hair-bearing skin and avoid conspicuous alopecia at the donor site. Because the flap remains vascularly connected rather than being fully detached and reattached, this is coded as Transfer rather than Replacement or Supplement. Documentation should describe both the defect being closed and the donor flap's origin, since the procedure inherently involves two adjacent sites on the same body part.

Approach: External

External approach applies to procedures performed directly on the skin or mucous membrane, or on an accessible body surface, without any instrumentation passing through a puncture or orifice. It covers things like manual reduction of a fracture or excision of a skin lesion. It differs from Via Natural or Artificial Opening in that no internal passage is entered at all, only the exposed surface.
