Procedure Overview

Venous ultrasonography, commonly known as a venous duplex scan, uses high-frequency sound waves rather than radiation to create real-time pictures of blood vessels and to measure how blood is flowing through them. A handheld transducer is pressed against the skin over the vein of interest, and the returning echoes are converted into a moving grayscale image, often paired with color or spectral Doppler that shows the direction and speed of flow.

This is the workhorse test for suspected deep vein thrombosis in the arms and legs because it is fast, painless, and can be repeated at the bedside without exposing the patient to contrast or radiation. It is also used to evaluate varicose veins and reflux before vein-ablation procedures, to check the patency of a vein before it is harvested for bypass surgery, and to confirm that a central line or dialysis access graft is functioning properly.

Because the study is examiner-dependent, the sonographer typically compresses the vein at intervals along its length; a vein that fails to collapse under gentle pressure indicates a clot inside it.

Jugular Veins, Left

The left jugular veins, primarily the internal and external jugular veins on the left side of the neck, are evaluated sonographically for thrombosis, catheter-related complications, and compressive pathology from adjacent structures such as enlarged lymph nodes or thyroid masses. Left internal jugular access is used less frequently than the right due to its more tortuous course and proximity to the thoracic duct, but ultrasound of this vein remains important for confirming patency prior to line placement and for assessing suspected thrombosis in patients presenting with left-sided neck swelling or a history of ipsilateral catheterization. Compressibility under transducer pressure and normal respirophasic flow are the key sonographic criteria used to exclude thrombus, distinguishing a patent from an occluded segment.

Coders assign a code from this family when the report documents a real-time sound-wave study of a specific venous segment, such as the femoral, popliteal, or jugular vein, rather than a generalized "leg ultrasound." The report should identify laterality when applicable and whether Doppler flow information was part of the study, since some facilities separate duplex (imaging plus Doppler) from simple B-mode imaging.

A frequent mistake is coding to a broad extremity value when the documentation actually supports a more specific named vein, which changes the body part character.
