Clinical Causes: Immune system attack on nerve fibers, leading to inflammation and damage. Chronic inflammation affecting peripheral nerves, especially in the limbs. Vasculitis caused by rheumatoid disease, leading to reduced blood flow to nerves. Potential neurotoxic effects of rheumatoid medications in some cases. Degeneration of nerve tissue due to longstanding joint disease and inflammation.

Key Symptoms: Numbness or tingling sensations in the affected areas, especially in the fingers and hand. Weakness in the muscles of the hand or wrist, impairing grip strength. Pain, which may be burning, stabbing, or throbbing, often worse at night. Loss of coordination or fine motor skills, making tasks like buttoning a shirt difficult. Swelling, stiffness, and pain in the left wrist characteristic of rheumatoid arthritis. Symptoms may worsen with activity and improve with rest. Other signs of nerve involvement may include muscle wasting or diminished reflexes.

Diagnosis Path: Medical history review focusing on joint symptoms and neurological complaints. Physical examination to evaluate joint swelling, tenderness, muscle strength, and nerve reflexes. Laboratory tests such as rheumatoid factor (RF), anti-CCP antibodies, and inflammatory markers (ESR, CRP). Electromyography (EMG) and nerve conduction studies to assess nerve function and locate nerve damage. Imaging studies like X-ray or MRI to evaluate joint inflammation and rule out other causes. Additional assessments may include nerve biopsy or blood tests to identify underlying causes of nerve involvement.

Treatment Protocols: Disease-modifying antirheumatic drugs (DMARDs), such as methotrexate, to control rheumatoid arthritis. Biologic therapies targeting specific immune system components to reduce inflammation. Pain management with nonsteroidal anti-inflammatory drugs (NSAIDs) or corticosteroids. Physical therapy to improve joint flexibility, strength, and nerve function. Occupational therapy to assist in adapting daily activities and improve quality of life. Symptomatic treatments like nerve pain medications or supplements to support nerve health. Regular monitoring to assess disease activity, manage side effects of medications, and adjust treatment protocols.
