Endometriosis of pleura

Clinical Classification Guidelines

Medical Intelligence & Overview

Endometriosis of the pleura is a rare condition where tissue similar to the lining of the uterus (endometrial tissue) is found within the pleural cavity, which surrounds the lungs. This abnormal tissue responds to hormonal changes, leading to symptoms that can significantly impact respiratory health. Although it shares characteristics with more common forms of endometriosis, its presence in the pleura makes it a unique and challenging condition to diagnose and treat.

Causes & Symptoms

Clinical Causes: Hormonal fluctuations, especially estrogen levels, that stimulate ectopic endometrial tissue growth Multiple pregnancies or cycles that may increase the risk of ectopic tissue developing outside the uterus Previous thoracic surgeries or trauma, which could facilitate the spread of endometrial cells to the pleura Genetic predisposition, although specific genetic factors remain under investigation Lymphatic or vascular dissemination of endometrial cells from the uterus to the pleural cavity

Key Symptoms: Chest pain, often sharp and localized, that may worsen during menstrual periods Recurrent pleural effusions or fluid buildup around the lungs, leading to shortness of breath Cough, which can be persistent or dry Hemoptysis (coughing up blood) in severe cases, especially during menstruation Signs of pneumothorax (collapsed lung), such as sudden chest pain and difficulty breathing Occasional fatigue or general malaise related to ongoing respiratory issues

Diagnostic & Treatment

Diagnosis Path: Detailed medical history focusing on menstrual patterns and respiratory symptoms Imaging tests such as chest X-rays or CT scans to identify pleural effusion, lung collapse, or other abnormalities Thoracentesis, a procedure to remove and analyze pleural fluid for the presence of endometrial tissue or associated blood Video-assisted thoracoscopic surgery (VATS) to directly visualize the pleura and obtain tissue biopsies for histopathological confirmation Histological examination of biopsy samples showing endometrial glands and stroma, often with evidence of hemorrhage

Treatment Protocols: Hormonal therapy such as gonadotropin-releasing hormone (GnRH) agonists to suppress ovarian function and reduce endometrial activity Oral contraceptives to regulate hormonal fluctuations and decrease endometrial tissue response Progestins or other hormonal agents to suppress ectopic tissue growth Surgical interventions, including thoracoscopy or open surgery, to remove endometrial deposits and repair pleural damage Management of recurrent pleural effusions with thoracentesis or pleurodesis to prevent fluid accumulation Supportive care, including pain management and respiratory therapy as needed

Reimbursement claims with a date of service on or after October 1, 2015 require the use of ICD-10-CM codes.
