Procedure Overview

Destruction procedures in this family eliminate abnormal or diseased tissue of the skull or facial bones using energy, heat, cold, chemicals, or other destructive means, without physically cutting the tissue out of the body. This approach is used for conditions such as ablating a small bony lesion, treating certain benign facial bone tumors, or eradicating infected or necrotic bone tissue in place using techniques like radiofrequency ablation or chemical cauterization.

Because the tissue is destroyed rather than excised and sent for pathology, this method is generally chosen when a definitive tissue diagnosis is already known or when removing the material intact is not necessary or feasible. It offers a less invasive alternative to open excisional surgery for appropriate lesions.

Parietal Bone, Right

The right parietal bone is one of the paired plates forming the lateral and superior walls of the cranial vault, directly overlying the parietal lobe and the superior sagittal sinus at the midline. Destruction of this bone is performed when abnormal tissue - such as an infiltrative lesion, osteomyelitic bone, or a vascular malformation involving the calvaria - is eradicated in place using thermal, chemical, or other destructive energy without physically removing bone from the body. Because the parietal bone is thin relative to other cranial bones and sits close to dural venous structures, precise control of the energy applied is important to limit heat or chemical spread toward the brain surface. Laterality must be documented accurately, as the right and left parietal bones are coded separately.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

A coder should look for language indicating the abnormal tissue was ablated, cauterized, fulgurated, or otherwise obliterated in place, with no portion of it removed from the body for further examination. The operative note should specify the destructive method used and the precise bony structure treated. A common mistake is applying Destruction when the physician actually excised the lesion and sent a specimen to pathology, which instead calls for Excision, or when the entire structure was removed, which may point to Resection.
