The left femoral vein carries deep venous drainage from the left leg toward the pelvis and is a well-recognized site for chronic obstruction following deep vein thrombosis, particularly when combined with iliac compression from May-Thurner physiology. Balloon dilation of this vessel addresses residual scarring or synechiae that narrow the lumen after thrombus organization, aiming to restore unobstructed outflow and reduce post-thrombotic symptoms. The left femoral vein's relationship to the profunda femoris confluence means operators must take care that dilation does not compromise flow at this junction. Because left-sided iliofemoral disease often extends across multiple contiguous segments, this vein is frequently treated in the same session as the adjacent iliac vein, and each treated segment should be coded separately to reflect the full extent of the intervention.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
