Surgeons excise tissue from the shoulder, elbow, wrist, or finger joints to relieve pain, restore range of motion, or send a tissue sample for diagnosis when imaging alone cannot explain a patient's symptoms.

Common reasons for this type of procedure include synovectomy for rheumatoid or inflammatory arthritis, debridement of a labral or cartilage tear, or removal of an osteophyte that is blocking joint movement. Because only a portion of the structure is taken and the joint itself is not replaced or reconstructed, patients typically recover function through physical therapy rather than needing an implant or prosthesis afterward.

The right carpal joint refers to the articulations among the eight small carpal bones that form the intricate midcarpal and intercarpal linkages of the wrist. Excision at this level is generally performed to remove degenerated cartilage, a bone spur, or diseased synovial tissue causing pain or restricted motion, often in the setting of osteoarthritis, avascular necrosis of a carpal bone, or chronic ligamentous instability. Because several small joints exist between adjacent carpal bones, precise identification of which articulation was addressed is important for accurate documentation, distinguishing this from procedures on the radiocarpal joint itself. The tight packing of bones and interosseous ligaments in this region demands meticulous technique to preserve the surrounding structures while removing only the targeted portion of joint tissue, since excessive disruption can compromise carpal alignment and function.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures.
