Procedure Overview

Fragmentation refers to breaking up a stone or other solid mass inside the urinary tract into smaller pieces without removing the pieces during the same maneuver. Shockwave lithotripsy (ESWL) delivered from outside the body and laser or ultrasonic lithotripsy performed through a ureteroscope are the two most common ways this is done for kidney and ureteral stones, and occasionally bladder stones. The goal is to reduce a stone too large to pass into fragments small enough to be passed naturally or removed with a separate instrument.

Bladder

The bladder is a distensible reservoir where stones may form primarily, from stagnant urine in cases of outlet obstruction or neurogenic dysfunction, or arrive secondarily after migrating down from the upper urinary tract. Because the bladder's capacity allows larger calculi to accumulate before symptoms prompt evaluation, cystolithiasis stones can be considerably bigger than those found in the ureters, sometimes requiring mechanical, ultrasonic, or laser cystolitholapaxy to break them into fragments suitable for irrigation or suction removal through a cystoscope. The procedure targets the calculus itself rather than the bladder wall, so no tissue is excised or repaired unless a separate root operation addresses an underlying anatomic cause such as an enlarged prostate.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

Documentation should describe the mechanism used to break the stone (shockwave, laser, ultrasonic) and the location - kidney, ureter, or bladder - since the code is specific to the body part where fragmentation occurred. A common coding pitfall is missing a second code for Extirpation when the operative note also describes basket retrieval of fragments in the same session, since fragmentation and extraction of the resulting pieces are captured as two separate root operations rather than one combined code.
