Procedure Overview
Fragmentation refers to breaking up a stone or other solid mass inside the urinary tract into smaller pieces without removing the pieces during the same maneuver. Shockwave lithotripsy (ESWL) delivered from outside the body and laser or ultrasonic lithotripsy performed through a ureteroscope are the two most common ways this is done for kidney and ureteral stones, and occasionally bladder stones. The goal is to reduce a stone too large to pass into fragments small enough to be passed naturally or removed with a separate instrument.

Ureter, Right
The right ureter is a narrow muscular tube prone to trapping calculi at its natural constrictions, particularly the ureteropelvic and ureterovesical junctions, where an impacted stone can cause severe colic and obstruct drainage from the kidney. Fragmentation of a ureteral stone is most often performed with a laser or pneumatic lithotripter passed through a ureteroscope, breaking the calculus into fragments small enough to migrate to the bladder or be basket-extracted in a related step. Because the ureter's caliber is so limited, even modest edema from instrumentation can cause temporary obstruction, which is why a stent is frequently placed afterward. The procedure note should make clear that the tube's wall was not incised or resected during fragmentation.
Approach: External
External approach applies to procedures performed directly on the skin or mucous membrane, or on an accessible body surface, without any instrumentation passing through a puncture or orifice. It covers things like manual reduction of a fracture or excision of a skin lesion. It differs from Via Natural or Artificial Opening in that no internal passage is entered at all, only the exposed surface.
