Procedure Overview
Fragmentation refers to breaking up a stone or other solid mass inside the urinary tract into smaller pieces without removing the pieces during the same maneuver. Shockwave lithotripsy (ESWL) delivered from outside the body and laser or ultrasonic lithotripsy performed through a ureteroscope are the two most common ways this is done for kidney and ureteral stones, and occasionally bladder stones. The goal is to reduce a stone too large to pass into fragments small enough to be passed naturally or removed with a separate instrument.

Urethra
The urethra is the terminal conduit for urine and, less commonly, semen, and its narrow diameter means that even a small calculus passing from the bladder can become lodged and cause acute retention. Fragmentation of a urethral stone is usually performed endoscopically with a cystoscope, applying laser or mechanical energy to break the obstruction into pieces that can be flushed out or gently extracted without incising the urethral mucosa. Because urethral anatomy differs substantially between male and female patients, with the male urethra being longer and traversing the prostate and penile shaft, the location of impaction and the instrumentation angle vary accordingly. Documentation should confirm that only the calculus, not the urethral wall, was disrupted.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
