Procedure Overview
Fragmentation involves breaking solid matter within the eye into smaller pieces without removing that matter from the body. In ophthalmology, this most often applies to breaking up a dense cataract using ultrasonic energy so the resulting fragments can be more easily managed, or to breaking apart a hardened deposit that would otherwise be too large to remove intact.
The purpose of fragmentation is preparatory: it makes the subsequent removal safer and less traumatic to surrounding eye tissue. Because the eye's internal structures are delicate, breaking material into smaller, more manageable pieces reduces the mechanical stress needed to clear it out.

The vitreous is the clear, gel-like substance filling the posterior chamber of the left eye between the lens and the retina, giving the globe its shape and providing an optically clear medium for light transmission. Fragmentation of the vitreous is performed when it has become clouded by hemorrhage, dense floaters, or opacified debris that obstructs vision or the surgeon's view of the retina, or when strands are causing traction on the retinal surface. Because vitreous has a delicate collagen and hyaluronic acid framework rather than solid tissue, mechanical or ultrasonic breakup is used to reduce it to fragments that can then be removed or reabsorbed without a separate extraction step in the same encounter. Documentation should specify that only physical disruption occurred, distinguishing this from procedures that also remove the fragmented material.
Approach: External
External approach applies to procedures performed directly on the skin or mucous membrane, or on an accessible body surface, without any instrumentation passing through a puncture or orifice. It covers things like manual reduction of a fracture or excision of a skin lesion. It differs from Via Natural or Artificial Opening in that no internal passage is entered at all, only the exposed surface.
