Procedure Overview
Fragmentation involves breaking solid matter within the eye into smaller pieces without removing that matter from the body. In ophthalmology, this most often applies to breaking up a dense cataract using ultrasonic energy so the resulting fragments can be more easily managed, or to breaking apart a hardened deposit that would otherwise be too large to remove intact.
The purpose of fragmentation is preparatory: it makes the subsequent removal safer and less traumatic to surrounding eye tissue. Because the eye's internal structures are delicate, breaking material into smaller, more manageable pieces reduces the mechanical stress needed to clear it out.

Vitreous, Right
The vitreous in the right eye is a clear, gel-like body filling the space between the lens and retina, providing structural support to the globe and maintaining contact with the retinal surface. Fragmentation of the vitreous breaks up this gel in place, most often using an ultrasonic or laser probe, to address conditions like dense vitreous hemorrhage, opacities, or floaters that obstruct vision, without necessarily removing the material afterward as a separate procedure would. Because the vitreous is intimately attached to the retina at the base and around the optic disc and macula, fragmentation must be performed carefully to avoid traction that could cause retinal tears or detachment.
