Procedure Overview

This family covers procedures that place a nonbiological device into the airway or chest to support breathing or monitor respiratory status, without removing or replacing any of the patient's own tissue. The most familiar examples are placing an endotracheal tube through the mouth or nose into the trachea to secure an airway during surgery or critical illness, and inserting a tracheostomy tube through a surgical opening in the neck for patients who need longer-term airway support. A monitoring device may also be placed in the airway or lung to track pressures or gas exchange over time. These procedures are performed when a patient cannot maintain their own airway, needs mechanical ventilation, or requires ongoing respiratory monitoring, ranging from emergency intubation to planned tracheostomy for chronic ventilator dependence.

Diaphragm

The diaphragm is the dome-shaped muscle separating the thoracic and abdominal cavities, and device insertion into it is most commonly performed to implant electrodes for diaphragmatic or phrenic nerve pacing in patients with high cervical spinal cord injury, central hypoventilation syndrome, or diaphragm paralysis requiring an alternative to mechanical ventilation. Electrodes are placed laparoscopically or thoracoscopically directly into the muscle at motor point locations identified by intraoperative stimulation mapping, then connected to an external or implantable pulse generator. Because the diaphragm is in constant motion and adjacent to the pericardium, liver, and spleen, precise electrode placement and fixation are critical to avoid dislodgement or injury to neighboring structures, and documentation should identify the specific hemidiaphragm and electrode type used.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

Device: Diaphragmatic Pacemaker Lead

A diaphragmatic pacemaker lead is the electrode component of a phrenic nerve or diaphragmatic pacing system, implanted to electrically stimulate the diaphragm and induce breathing in patients with chronic ventilatory failure, such as high cervical spinal cord injury. It differs from cardiac pacemaker leads in that its target is the diaphragm or phrenic nerve, not the heart, and it is reported with the respiratory rather than cardiovascular body system.

Coding & Documentation

Coders assign from this family when documentation describes placing a device without disturbing or excising native anatomy, and the device stays behind after the procedure ends. Operative notes should specify the device type (endotracheal tube, tracheostomy tube, monitoring device) and the body part entered, since that determines the qualifier and body part value. A frequent error is coding a tracheostomy tube exchange or routine reinsertion as Insertion when it should reflect the original placement versus a change, or confusing airway securement during anesthesia (not separately coded) with a therapeutic tracheostomy tube placement. Coders also sometimes miss that a percutaneous versus open tracheostomy approach changes the approach character even though the root operation stays the same.
