Procedure Overview

The most familiar examples are placing an endotracheal tube through the mouth or nose into the trachea to secure an airway during surgery or critical illness, and inserting a tracheostomy tube through a surgical opening in the neck for patients who need longer-term airway support. A monitoring device may also be placed in the airway or lung to track pressures or gas exchange over time. These procedures are performed when a patient cannot maintain their own airway, needs mechanical ventilation, or requires ongoing respiratory monitoring, ranging from emergency intubation to planned tracheostomy for chronic ventilator dependence.

Diaphragm
The diaphragm is the dome-shaped muscle separating the thoracic and abdominal cavities, and device insertion into it is most commonly performed to implant electrodes for diaphragmatic or phrenic nerve pacing in patients with high cervical spinal cord injury, central hypoventilation syndrome, or diaphragm paralysis requiring an alternative to mechanical ventilation. Electrodes are placed laparoscopically or thoracoscopically directly into the muscle at motor point locations identified by intraoperative stimulation mapping, then connected to an external or implantable pulse generator. Because the diaphragm is in constant motion and adjacent to the pericardium, liver, and spleen, precise electrode placement and fixation are critical to avoid dislodgement or injury to neighboring structures, and documentation should identify the specific hemidiaphragm and electrode type used.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
Device: Monitoring Device
Monitoring Device designates a device left in place to measure or track a physiologic parameter, such as an implanted pressure or activity sensor. It differs from Drainage Device, which removes fluid or air, by instead collecting data for ongoing clinical assessment without evacuating any substance from the body.
