Procedure Overview
Procedures in this family involve looking at or manually examining a respiratory structure, such as the trachea, bronchi, lungs, or pleural cavity, without cutting out, repairing, or altering tissue. Bronchoscopy is the most common example, where a flexible scope is passed through the airway to visually assess the lining of the trachea and bronchi, check for tumors, bleeding, or foreign bodies, or evaluate an unexplained cough or abnormal imaging finding. Mediastinoscopy and thoracoscopy performed purely to look inside the chest also fall here when no additional therapeutic action is taken during the same encounter.

Lung, Left
Inspection of the left lung involves direct visualization, typically via thoracoscopy, of its two lobes and pleural surface to evaluate findings such as a suspicious nodule, unexplained effusion, or diffuse disease not adequately characterized by imaging alone. The left lung's single oblique fissure separating the upper and lower lobes, along with the cardiac notch accommodating the heart, serve as anatomic landmarks during the exam. Because the left lung lies adjacent to the heart and great vessels, the surgical approach and instrument angling must account for this proximity, and any abnormal areas identified are documented by lobe and surface location to guide subsequent biopsy or resection decisions.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
Coding & Documentation
This root operation is assigned only when the sole intent and outcome of the procedure was exploration, not when inspection is a routine preliminary step before a biopsy, resection, or repair performed in the same operative session. Documentation should clearly state that the airway or cavity was examined and describe what was or was not found, with no separate destructive, excisional, or reconstructive act. A common coding mistake is assigning Inspection in addition to a therapeutic root operation performed during the same bronchoscopy, when PCS guidelines direct that inspection of a body part is not coded separately if a more definitive procedure is done on that same body part during the same operative episode.
Commonly Confused With
Inspection is frequently confused with Excision or Extirpation performed via bronchoscopy, such as when a biopsy or foreign body removal happens during the same scope pass; in those cases the therapeutic root operation is coded instead of, not in addition to, Inspection. It is also distinct from diagnostic imaging procedures, which use radiologic technology rather than direct visualization or palpation to evaluate the airway.
