A machine briefly passes ionizing radiation through the body, and tissues of different density - bone, air, soft tissue - absorb it differently, creating a flat, two-dimensional image on a plate or digital detector.
These studies are ordered for a wide range of reasons: checking for fractures after an injury, evaluating chest symptoms like cough or shortness of breath, looking for free air or obstruction in the abdomen, or getting a quick baseline view before deciding whether more detailed imaging is needed. The exam itself takes only moments and requires no preparation in most cases, which makes it a practical first step in many diagnostic pathways.

Upper Extremity
Radiographic evaluation of the upper extremity covers the shoulder, humerus, elbow, forearm, wrist, and hand, and is most commonly ordered after a fall onto an outstretched hand, a direct blow, or for evaluation of chronic joint pain or suspected arthritis. Because many upper extremity fractures, such as scaphoid or radial head injuries, can be subtle or occult on initial films, dedicated oblique or stress views are frequently added, and a normal initial study sometimes prompts a short-interval repeat film once soft tissue swelling has had time to make a fracture line more visible. The wrist and hand in particular contain numerous small carpal bones that overlap on standard views, so multiple angles are typically required to adequately clear each bone of suspected injury.
Coding & Documentation
Coders need documentation specifying the exact anatomical region imaged and how many views were taken, since the number of views can affect the qualifier chosen. The report should distinguish a single view from a series, and note whether contrast was used, because plain radiography without contrast is coded differently from a contrast-enhanced study.
A frequent mistake is defaulting to a generic region code when the documentation actually supports a more specific body part, or overlooking that a single order produced two separate studies (for example, chest and abdomen) that each need their own code. Commonly Confused With
This family is easily confused with fluoroscopy, since both use ionizing radiation on similar equipment; the difference is that plain radiography captures a single static image while fluoroscopy produces continuous real-time viewing. It is also distinct from CT of the same region - CT reconstructs many exposures into cross-sectional images, whereas plain film produces one flat projection.
