Procedure Overview
Release procedures free the liver, gallbladder, bile ducts, or pancreas from an abnormal band of scar tissue, adhesion, or other constricting structure that is restricting its movement or function, without cutting into or removing any of the organ itself. The surgeon divides or removes only the confining material, leaving the organ intact.
This is commonly needed when adhesions from prior abdominal surgery, inflammation, or chronic pancreatitis have bound an organ to surrounding structures, causing pain, obstruction, or impaired function. Releasing these constraints can relieve symptoms and restore more normal anatomic mobility and drainage.

The common hepatic duct forms where the right and left hepatic ducts join just outside the liver, carrying all bile toward the cystic duct junction and on into the common bile duct. It can become narrowed by adjacent inflammation, a scarred surgical bed from prior cholecystectomy, or compression from enlarged periportal lymph nodes or tumor, producing obstructive jaundice even when the duct wall itself is structurally intact. Release frees this segment from surrounding scar tissue, fibrous bands, or an encasing mass without cutting into the duct lumen or excising the compressing structure, restoring free bile flow. Because the duct sits close to the portal vein and hepatic artery, this dissection is done carefully to avoid vascular injury, and the approach is often documented alongside intraoperative cholangiography confirming that flow through the duct has been restored.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
