Procedure Overview
Repair is the default root operation used when an endocrine gland has been damaged - by trauma, a surgical complication, or a structural defect - and needs to be restored to its normal anatomy and function using a method not captured by any more specific root operation. Common examples include suturing a lacerated adrenal gland or closing an inadvertent capsular tear during thyroid or parathyroid surgery.
Because Repair functions as PCS's catch-all for reconstructive work on a body part, it appears whenever a surgeon fixes torn, lacerated, or otherwise structurally compromised glandular tissue without performing a more precisely defined procedure like grafting, replacement, or a named reconstructive technique. It restores the gland's physical integrity so it can continue functioning in its normal location.

On the left side, the inferior parathyroid gland likewise arises from the third pharyngeal pouch but travels a longer descent path with the thymus, making it somewhat more prone to ectopic placement in the neck or superior mediastinum than its superior counterpart. Repair coding applies when the gland's capsule is torn, devascularized, or actively bleeding during dissection - commonly a complication noted while operating on the adjacent left thyroid lobe or during exploration for hyperparathyroidism - and the surgeon corrects the defect in place rather than removing the gland. Preserving even a fragment of viable, well-vascularized tissue is clinically important, since loss of all four parathyroid glands' function leads to permanent hypoparathyroidism, so documentation of a deliberate repair reflects an effort to protect endocrine reserve.
Approach: Percutaneous Endoscopic
Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
