Procedure Overview

This family involves putting in biological or synthetic material that physically takes over for a missing or damaged portion of the respiratory tract, most notably reconstruction of the trachea using graft material after a segment is removed for stenosis or tumor invasion. Unlike a lung transplant, which replaces the entire organ from a donor and is classified separately, these procedures typically address a segment of the airway wall.

Patients undergo these procedures when the trachea has been narrowed, weakened, or destroyed to the point that simple repair won't hold, often after prior injury from prolonged intubation, tumor removal, or a congenital defect. Diaphragm

The diaphragm is the dome-shaped musculotendinous partition separating the thoracic and abdominal cavities and functioning as the primary muscle of respiration, a role that makes any structural defect immediately consequential for ventilation. Replacement involves substituting a damaged or absent portion of the diaphragm, whether from tumor resection, traumatic rupture, or a large congenital or acquired hernia defect, with a prosthetic patch or biologic mesh sewn into the remaining native muscle and surrounding rib cage or costal margin. Because the diaphragm must withstand constant cyclical motion and pressure differentials between the chest and abdomen, the replacement material has to tolerate repeated stress without eroding into adjacent organs. Documentation should specify that a graft or mesh physically took over the anatomic role of the diaphragmatic tissue, as opposed to a suture repair of an existing defect.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
