Surgeons turn to this approach in situations such as reconstructing a badly damaged femoral or iliac vein after trauma, cancer resection that required removing a vein along with a tumor, or chronic venous disease that has destroyed a segment beyond salvage. The goal is to restore a functioning channel for venous return rather than simply tying off the vessel, which can be important for preventing long-term swelling and pressure buildup in the limb.

Because deep veins carry a large volume of blood at low pressure, these reconstructions are technically demanding and are typically reserved for cases where the alternative - vein ligation or leaving the segment untreated - would cause significant disability.

Splenic Vein

The splenic vein runs along the posterior pancreas and drains the spleen and portions of the stomach and pancreas into the portal system, making it vulnerable to compression or invasion by pancreatic tumors, pancreatitis-related thrombosis, or splenic pathology. Replacement of a diseased segment with a graft is a technically demanding step within larger pancreatic or splenic resections, undertaken when the vessel cannot simply be repaired or ligated without compromising portal inflow and risking left-sided portal hypertension. Its close anatomic relationship to the pancreatic tail and portal confluence means replacement is rarely an isolated operation, and documentation should reflect its role within the broader procedure, such as pancreatectomy with vascular reconstruction.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Device: Synthetic Substitute

Synthetic Substitute designates a device made from manufactured, non-biologic material, such as mesh or prosthetic components, used to replace or augment a body part. It is distinguished from the two tissue substitute categories by its artificial composition, which carries different considerations for integration and long-term durability than biologic grafts.
