Reposition procedures address ocular structures that are physically present and structurally intact but located in the wrong place, moving them to their normal or another suitable anatomic position. In the eye, this most often applies to strabismus surgery, where an extraocular muscle is detached and reattached at a different point on the sclera to correct misalignment, or to correction of a dislocated or subluxated natural lens. Eyelid procedures that reposition displaced tissue, such as correcting entropion or ectropion by relocating eyelid margin tissue, also fall into this category.
These procedures are performed to correct misalignment that causes double vision, poor depth perception, cosmetic concern, or corneal irritation from an abnormally positioned eyelid or lash line. Because the structure itself is not being removed or replaced, the surgical work centers on detachment, relocation, and reattachment.

The right extraocular muscles are the six muscles attaching to the globe that control eye movement and alignment, including the medial, lateral, superior, and inferior recti and the oblique muscles. The most frequent coding pitfall is applying the root operation Resection to a strabismus muscle "resection" procedure based on the clinical terminology alone, when the PCS definition actually calls for Reposition because the muscle remains attached and functional at its new site.
