Procedure Overview

Reposition procedures address ocular structures that are physically present and structurally intact but located in the wrong place, moving them to their normal or another suitable anatomic position. In the eye, this most often applies to strabismus surgery, where an extraocular muscle is detached and reattached at a different point on the sclera to correct misalignment, or to correction of a dislocated or subluxated natural lens. Eyelid procedures that reposition displaced tissue, such as correcting entropion or ectropion by relocating eyelid margin tissue, also fall into this category.

These procedures are performed to correct misalignment that causes double vision, poor depth perception, cosmetic concern, or corneal irritation from an abnormally positioned eyelid or lash line. Because the structure itself is not being removed or replaced, the surgical work centers on detachment, relocation, and reattachment.

Retinal Vessel, Left

The left retinal vessels carry blood to and from the retinal tissue, running along the inner retinal surface in a pattern radiating from the optic nerve head. Reposition applies when a retinal vessel is surgically displaced, such as during sheathotomy for branch retinal vein occlusion, where the surgeon separates the vein from an overlying compressive artery at a crossing point to restore normal blood flow and reduce associated retinal edema. The vessel remains intact and functional but is moved relative to adjacent tissue, distinguishing this from destruction or occlusion procedures used elsewhere in vascular retinal disease. Given the delicacy of retinal microsurgery, this root operation is typically documented only in specialized vitreoretinal cases and should be confirmed against explicit operative language describing vessel displacement rather than laser photocoagulation or injection therapy.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

Coders should assign Reposition when the operative note describes moving an existing structure, most classically the recession or resection of an extraocular muscle in strabismus surgery, though note that despite the term "resection" used clinically for tightening a muscle, this procedure is still coded as Reposition under ICD-10-PCS conventions since the muscle is not being cut out and discarded. Documentation should specify which muscle was moved and to what new location or by what amount.

The most frequent coding pitfall is applying the root operation Resection to a strabismus muscle "resection" procedure based on the clinical terminology alone, when the PCS definition actually calls for Reposition because the muscle remains attached and functional at its new site. Coders should also watch for cases involving multiple muscles operated on in one session, each requiring separate code assignment.
