Procedure Overview

Ultrasonography of the female reproductive system uses high frequency sound waves, not radiation, to create real time images of the uterus, ovaries, fallopian tubes, and surrounding pelvic structures. It is one of the most frequently used tools in gynecology and obstetrics because it is quick, does not require ionizing radiation, and can be performed either through the abdomen or with a probe placed in the vagina for a closer, more detailed view.

This type of imaging is ordered to investigate pelvic pain, abnormal bleeding, suspected fibroids or ovarian cysts, infertility evaluations, and to monitor a pregnancy at any stage, including checking fetal growth, position, and heartbeat. Because the images are captured in real time, the study can also assess blood flow through vessels using Doppler technique, which helps evaluate masses or placental function.

Most patients experience it as a routine, low-risk procedure, though a transvaginal exam requires some preparation, such as a full bladder for the abdominal portion or an empty bladder for the internal portion, depending on what the physician needs to see.

Bilateral fallopian tube ultrasonography evaluates both tubes in a single study, most commonly during a sonohysterosalpingogram in which saline or an ultrasound contrast agent is instilled through the cervix to confirm patency on each side as part of an infertility workup. This approach avoids the ionizing radiation of a conventional radiographic hysterosalpingogram while still allowing assessment of tubal spill and any hydrosalpinx or peritubal adhesion affecting either tube. Bilateral imaging is also used when pelvic inflammatory disease or suspected tubo-ovarian abscess could involve both adnexa, since unilateral disease often has contralateral effects. Because the fallopian tubes are otherwise difficult to distinguish from surrounding bowel and peritoneal fat, this code specifically denotes a study designed to interrogate both tubes together rather than incidental visualization during a routine pelvic scan.

Contrast: Other Contrast

Other Contrast captures imaging studies using a contrast medium that is neither high- nor low-osmolar iodinated agent, such as barium sulfate for gastrointestinal studies or gadolinium-based agents for certain applications outside standard MRI classification within this axis. It is used when the contrast administered does not fit the osmolarity-based iodinated categories, distinguishing it from those specific classes and from studies performed with no contrast.

This family is often confused with Fluoroscopy or Plain Radiography of the same region when a report casually says imaging without stating the modality clearly. It can also be mixed up with CT or MRI studies of the pelvis ordered for similar clinical reasons, such as evaluating a mass; the distinction again rests strictly on which imaging technology generated the images, not on why the study was ordered.
