Procedure Overview

Ultrasonography of the female reproductive system uses high frequency sound waves, not radiation, to create real time images of the uterus, ovaries, fallopian tubes, and surrounding pelvic structures. It is one of the most frequently used tools in gynecology and obstetrics because it is quick, does not require ionizing radiation, and can be performed either through the abdomen or with a probe placed in the vagina for a closer, more detailed view.

This type of imaging is ordered to investigate pelvic pain, abnormal bleeding, suspected fibroids or ovarian cysts, infertility evaluations, and to monitor a pregnancy at any stage, including checking fetal growth, position, and heartbeat. Because the images are captured in real time, the study can also assess blood flow through vessels using Doppler technique, which helps evaluate masses or placental function.

Most patients experience it as a routine, low-risk procedure, though a transvaginal exam requires some preparation, such as a full bladder for the abdominal portion or an empty bladder for the internal portion, depending on what the physician needs to see.

Ovary, Left

The left ovary sits within the ovarian fossa on the left pelvic sidewall and is assessed by ultrasound using the same criteria applied to its counterpart, including size, follicle count, and echotexture of any mass. Left ovarian ultrasonography is frequently indicated for lateralized pelvic pain concerning for torsion, hemorrhagic cyst, or endometrioma, and for monitoring follicular response during ovulation induction cycles. Doppler evaluation of flow to the left ovary is particularly important when torsion is suspected, since absent or diminished flow supports urgent surgical evaluation, though normal flow does not entirely exclude intermittent torsion. As with the right side, prior surgical history affecting the left adnexa should be documented, since a surgically absent ovary cannot be imaged. This code applies when the left ovary alone, rather than both ovaries, is the target of the examination.

Contrast: Other Contrast

Other Contrast captures imaging studies using a contrast medium that is neither high- nor low-osmolar iodinated agent, such as barium sulfate for gastrointestinal studies or gadolinium-based agents for certain applications outside standard MRI classification within this axis. It is used when the contrast administered does not fit the osmolarity-based iodinated categories, distinguishing it from those specific classes and from studies performed with no contrast.

A code from this family is assigned when documentation confirms sound wave based imaging, not CT, MRI, or x-ray, was used, and specifies the body part examined, such as uterus, ovary, or pregnant uterus for obstetric studies. Coders should look for the word ultrasound or sonography explicitly in the imaging report rather than assuming it from the ordering department. A common error is defaulting to a generic pelvic ultrasound code when the documentation actually specifies a more precise structure, such as the ovary alone, or confusing a pregnancy related ultrasound with a routine gynecologic study, since obstetric ultrasounds use distinct body part values under the pregnant uterus.

Another frequent mistake is coding a Doppler ultrasound as if it were a separate root operation; Doppler flow studies still fall under Ultrasonography in this system.

Commonly Confused With

This family is often confused with Fluoroscopy or Plain Radiography of the same region when a report casually says imaging without stating the modality clearly. The deciding factor is always the energy source: ultrasonography uses sound waves, while fluoroscopy and radiography use ionizing radiation, so the coder must verify the modality name in the report before assigning either. It can also be mixed up with CT or MRI studies of the pelvis ordered for similar clinical reasons, such as evaluating a mass; the distinction again rests strictly on which imaging technology generated the images, not on why the study was ordered.
