Procedure Overview

Venous ultrasonography, commonly known as a venous duplex scan, uses high-frequency sound waves rather than radiation to create real-time pictures of blood vessels and to measure how blood is flowing through them. A handheld transducer is pressed against the skin over the vein of interest, and the returning echoes are converted into a moving grayscale image, often paired with color or spectral Doppler that shows the direction and speed of flow.

This is the workhorse test for suspected deep vein thrombosis in the arms and legs because it is fast, painless, and can be repeated at the bedside without exposing the patient to contrast or radiation. It is also used to evaluate varicose veins and reflux before vein-ablation procedures, to check the patency of a vein before it is harvested for bypass surgery, and to confirm that a central line or dialysis access graft is functioning properly.

Because the study is examiner-dependent, the sonographer typically compresses the vein at intervals along its length; a vein that fails to collapse under gentle pressure indicates a clot inside it.

Renal Vein, Right

The right renal vein is a short vessel draining directly from the kidney into the inferior vena cava at a nearly perpendicular angle, a course that makes it more compact to interrogate sonographically than its longer left-sided counterpart but also more prone to being missed on a cursory scan. Ultrasonography of this vein is performed to evaluate for renal vein thrombosis, which can occur with nephrotic syndrome, renal cell carcinoma, or hypercoagulable states, and to assess flow direction and velocity when renal vein compression or tumor thrombus is suspected. Color Doppler with spectral waveform analysis helps distinguish sluggish but patent flow from true luminal occlusion at this junction with the cava.
