O00.2

Ovarian pregnancy

Clinical Classification Guidelines

Medical Intelligence & Overview

Ovarian pregnancy is a rare type of ectopic pregnancy where a fertilized egg implants itself within the ovary instead of the uterus. This condition poses significant health risks and requires prompt medical attention. Recognizing the signs and understanding the causes can help in managing this uncommon but potentially dangerous pregnancy complication.

Causes & Symptoms

Clinical Causes: Abnormalities in the fallopian tubes that prevent the egg from reaching the uterus, causing it to implant in the ovary instead. Use of fertility treatments or assisted reproductive techniques, which might increase the likelihood of ectopic implantations. History of pelvic infections or inflammation that damage the reproductive organs, creating an environment conducive to abnormal implantation. Previous pelvic or abdominal surgeries that may affect normal anatomical structures. Introduction of fertilized eggs into the ovary due to abnormal fertilization processes.

Key Symptoms: Severe pain in the lower abdomen or pelvis, often on one side. Vaginal bleeding or spotting that may be light or heavy. Feeling of dizziness or fainting, which could indicate internal bleeding. Shoulder pain, which can be a sign of internal bleeding irritating the diaphragm. Unusual abdominal tenderness or swelling.

Diagnostic & Treatment

Diagnosis Path: Pelvic examination to assess abdominal tenderness and any palpable mass. Transvaginal ultrasound to visualize the ovaries, fallopian tubes, and uterus for signs of ectopic implantation. Blood tests measuring levels of human chorionic gonadotropin (hCG), which may be lower or rise abnormally in ectopic pregnancies. Diagnostic laparoscopy, a minimally invasive surgical procedure to directly view the reproductive organs and confirm the presence of an ovarian pregnancy.

Treatment Protocols: Medical management with methotrexate, a medication that terminates the pregnancy by stopping cell growth. Surgical intervention, typically via laparoscopy, to remove the ectopic tissue and preserve ovarian function. In cases of significant bleeding or rupture, emergency surgery may be necessary to control bleeding and remove damaged tissue. Post-treatment monitoring of hCG levels to ensure complete resolution of the pregnancy tissue.
