Operation Definition
Taking out or off a device from a body part and putting back an identical or similar device in or on the same body part without cutting or puncturing the skin or a mucous membrane
Procedure Overview
Change procedures involve taking a device off or out of the skin or breast and putting an identical or similar device back in the same spot, all without cutting or puncturing the skin. The everyday example is exchanging a wound vacuum-assisted closure dressing, a pressure dressing, or an external drain apparatus placed over a wound or surgical site.
These exchanges happen routinely during wound care, whether in a clinic, wound care center, or hospital setting, and are necessary to keep therapy working effectively, since dressings and external devices lose suction, become soiled, or need periodic replacement to continue promoting healing. Because no incision or new puncture is involved, this is one of the least invasive procedure types in the skin and breast body system.
Anatomy & Axis Detail
Breast, Right
Change of a device in the right breast describes routine, non-incisional replacement of a device situated in that breast, such as a tissue expander port cap, a drain, or a temporary catheter used during reconstruction or abscess management. The right breast is coded distinctly from the left because laterality affects both anatomic tracking and later comparison across encounters, particularly in patients undergoing staged reconstruction after mastectomy where expanders are swapped for permanent implants over multiple visits. Since the device is taken out and a similar one put back without disrupting the skin envelope, this differs sharply from Removal or Insertion performed as separate incisional procedures, and documentation should confirm the exchange was accomplished through the existing access point.
Approach: External
External approach applies to procedures performed directly on the skin or mucous membrane, or on an accessible body surface, without any instrumentation passing through a puncture or orifice. It covers things like manual reduction of a fracture or excision of a skin lesion. It differs from Via Natural or Artificial Opening in that no internal passage is entered at all, only the exposed surface.
Device: Drainage Device
Drainage Device denotes a device such as a tube or catheter left in place to remove fluid, blood, or air from a body part or cavity following a procedure. It is distinguished from Monitoring Device, which senses and records physiologic data rather than evacuating substances from the body.
Coding & Documentation
Coders need documentation confirming the device was removed and replaced through an existing opening rather than through a new cut or puncture, and that the replacement device serves the same function as the one removed. A common mixup happens when a wound vac dressing change is bundled into routine wound care documentation without being called out as a distinct procedure, causing it to be missed entirely, or conversely counted separately when it was simply part of ongoing dressing changes not meant to be coded as a procedure. Coders should also verify that the device is genuinely being swapped for an equivalent one, since replacing a simple dressing with a different type of device, such as converting standard dressing care to a wound vac, may point to a different, non-Change procedure.
